We are aware that some parents and staff members have experienced difficulty accessing the Daily Health Survey recently. For those using a Samsung Mobile Device, the form will not operate properly in the default Samsung browser. You will need to use the Chrome browser on the device, or use a different device, to complete the form. To update your default browser to Chrome follow the steps below. Samsung updated its browser recently, making it incompatible with Google Forms.
